Have the Collection Agency Annual Renewal form in other digital formats

As a licensee we are required to use the DOF forms to submit information. However, the PDF provided can not be typed in and the boxes are very small to fill in by hand when printed. In addition, not everyone uses Excel or has the current version to use the file type provided (.xls vs .xlsx). A PDF with form fields would be great.
